First European Large-Scale Clinical Study Shows AI Scribes Reduce Documentation Time After 375,000 Medical Notes Created

Stockholm, Sweden, December 2025: A first of its kind large-scale study of AI medical scribes conducted in Sweden has demonstrated that the technology addresses administrative burden in clinical practice. The dataset consisted of over 375,000 medical notes created using Tandem Health’s AI Scribe by 1,295 clinicians at Capio, the Nordic region’s largest private health provider, across primary, secondary and hospital care.

The mixed-method study selected the most relevant notes from the 375,000-note dataset, complemented by a survey with responses from 177 fully onboarded professionals responsible for over 60,000 notes generated by Tandem. It found that the AI technology reduced estimated time spent on medical notes by 29%, from 6.69 minutes to 4.71 minutes per note.

The research addresses a significant evidence gap in AI deployment across European healthcare systems. Previous research on AI medical scribes has concentrated almost exclusively in United States health systems.

These findings now provide evidence that AI medical scribes can reduce administrative burden and improve clinician experience within European healthcare systems and under EU Medical Device Regulation frameworks.

In addition to significant time savings, the study documented measurable improvements in clinician wellbeing. Key findings include:

  • Clinicians on average felt 16% more present during patient consultations, with scores on a five-point scale increasing from 3.73 to 4.33
  • Clinicians reported a 30% reduction in stress from administrative tasks, with mean scores improving from 2.41 to 3.14 in terms of being able to work without feeling stressed by administrative tasks
  • Objectively measured median editing time was 93 seconds per note based on usage data.

Furthermore, adoption metrics indicated strong acceptance of AI technology by healthcare professionals:

  • 91% reported the technology was easy to use
  • 91% wanted to continue using it
  • 84% would recommend it to colleagues
  • Notably, 80% stated the scribe was an improvement for their profession.

These changes underscore that AI scribes can move the needle on clinician burnout, a critical factor driving workforce retention challenges.

The deployment at Capio spanned diverse clinical settings across primary and secondary care, and hospital care. General practitioners represented 66% of the 1,295 participating clinicians, and the remainder covered 43 professions and specialties, including physiotherapists, orthopaedic surgeons, psychologists, and nurses.

Many clinicians in the survey felt that they gained a third of their time back when using Tandem.

Healthcare professionals saw consistent time savings of two minutes per note. If this was scaled across Swedish healthcare, it could lead to a potential saving of over one million hours every year. For European healthcare systems facing acute workforce shortages and rising costs, that capacity gain could make a real difference at scale, addressing wait times, clinician burnout and care access.”

European healthcare operates under distinct regulatory requirements, and the technology utilised in the study holds CE marking as a Class I medical device under the EU Medical Device Regulation. The Capio deployment provides a foundation of evidence for European health systems evaluating these technologies.

European clinicians work under strict regulatory frameworks, complex documentation standards, and a fragmented landscape of electronic health record systems – all of which shape how AI scribes must be designed and how they perform in practice.

“Our study sets an important foundation for regulators, policymakers, and health care operators to understand the technology and how it can be applied to address our challenges related to workforce shortages, working conditions, and productivity in our health care systems. Our data supports the hypothesis that AI assistants are perceived as beneficial for the profession and can support patient-centred care when they are developed and implemented in an appropriate way.”

The research combines quantitative selected usage data from over 375 000 generated notes with survey responses from 177 clinicians responsible for 60,000 notes. The study was preregistered on the Open Science Framework and adhered to EQUATOR guidelines for quality improvement research. The findings closely mirror results from the large Kaiser Permanente study in the United States.

Tandem Health, founded in Stockholm in 2023, now serves over 1,000 healthcare organisations across 11 European markets, including partnerships with Sweden’s Capio Ramsay Santé, Spain’s Recoletas, France’s Rothschild Foundation Hospital, and the UK’s NHS. Nearly half of Tandem’s 100-person team consists of healthcare professionals.
